TMUH Heart Failure Care Team Pioneeringly Wins Gold Plus Award from American Heart Association in Asia

On March 20th, 2024, the ?Heart Failure Care Team received the?from the American Heart Association (AHA), marking the first recognition in Asia. This award highlights TMUH¡¯s high-quality medical service and contribution to sustainable development.

Practicing the concept of a ¡°team care model,¡± the TMUH Heart Failure Care Team integrates services such as home-based remote physiological monitoring and rehabilitation. This model optimizes care for post-acute heart failure and extends care from passive treatment to disease prevention. Additionally, it establishes locally customized cross-disciplinary medical care procedures.?

The team, consisting of a wide range of medical professionals including cardiologists, surgeons, clinical pharmacists, dietitians, a rehabilitation team, and a remote service team, implements customized care plans. The cardiology team analyzes the cause and prognostic factors of the patient¡¯s condition by utilizing advanced three-dimensional and speckle tracking echocardiography. Alongside pharmacists and nutritionists, the care team conducts a comprehensive cardiopulmonary assessment, performs sarcopenia screening, and instructs on rehabilitation to ensure the full recovery of the patient.

The TMUH Heart Failure Care Team¡¯s commitment to high-quality and comprehensive medical care was recognized twice by the Joint Commission of Taiwan in 2019 and 2022. Meanwhile, the team actively participates in the Get-With-The-Guidelines (GWTG) Program organized by the AHA and ICHOM (The International Consortium for Health Outcomes Measurements), dedicating efforts to transforming TMUH into a smart hospital and green medical environment.
