On March 20th, 2024, the ?Heart Failure Care Team received the?from the American Heart Association (AHA), marking the first recognition in Asia. This award highlights TMUH’s high-quality medical service and contribution to sustainable development.

Practicing the concept of a “team care model,” the TMUH Heart Failure Care Team integrates services such as home-based remote physiological monitoring and rehabilitation. This model optimizes care for post-acute heart failure and extends care from passive treatment to disease prevention. Additionally, it establishes locally customized cross-disciplinary medical care procedures.?

The team, consisting of a wide range of medical professionals including cardiologists, surgeons, clinical pharmacists, dietitians, a rehabilitation team, and a remote service team, implements customized care plans. The cardiology team analyzes the cause and prognostic factors of the patient’s condition by utilizing advanced three-dimensional and speckle tracking echocardiography. Alongside pharmacists and nutritionists, the care team conducts a comprehensive cardiopulmonary assessment, performs sarcopenia screening, and instructs on rehabilitation to ensure the full recovery of the patient.

The TMUH Heart Failure Care Team’s commitment to high-quality and comprehensive medical care was recognized twice by the Joint Commission of Taiwan in 2019 and 2022. Meanwhile, the team actively participates in the Get-With-The-Guidelines (GWTG) Program organized by the AHA and ICHOM (The International Consortium for Health Outcomes Measurements), dedicating efforts to transforming TMUH into a smart hospital and green medical environment.
